APPLICATION FORM TO DATA CONTROLLER

1. General Explanations

Personal data owners (“Applicant”), defined as the data subject in the Personal Data Protection Law No. 6698 (“KVKK”), have been granted the right to make certain requests regarding the processing of their personal data pursuant to Article 11 of the KVKK.

The application form has been prepared to determine your relationship with Öztanık Bilişim İletişim Turizm İnşaat Ltd. Şti. (“Anka Hotel”), to determine your personal data processed by Anka Hotel completely, if any, and to respond to your relevant application accurately and within the legal period. In order to ensure the security of your personal data and prevent unlawful data transfer, additional information may be requested by Anka Hotel for identity and authority determination. In case the information provided by the Applicant is incorrect and/or not up-to-date or the requests are unauthorized, the responsibility regarding this issue belongs to the Applicant.

Pursuant to Article 7 of the Communiqué on Procedures and Principles of Application to the Data Controller, if the Applicant's application is to be answered in writing, no fee is charged for up to ten pages. A transaction fee of 1 Turkish Lira may be charged for each page over ten pages. If the response to the application is given on a recording medium such as CD, flash memory, a fee equivalent to the cost of the recording medium may be requested.

2. Scope of the Right of Application within the Framework of Article 11 of KVKK

The Applicant may apply to Anka Hotel and make requests on the following issues:

1) To learn whether their personal data is processed,

2) To request information if their personal data has been processed,

3) To learn the purpose of processing their personal data and whether they are used in accordance with their purpose,

4) To learn the third parties to whom their personal data is transferred domestically or abroad,

5) To request correction of their personal data if it is incomplete or incorrectly processed and to request notification of the transaction made in this scope to third parties to whom personal data is transferred,

6) To request the deletion, destruction or anonymization of their personal data if the reasons requiring processing are eliminated despite being processed in accordance with KVKK and other relevant law provisions, and to request notification of the transaction made in this scope to third parties to whom personal data is transferred,

7) To object to the emergence of a result against them by analyzing their processed data exclusively through automated systems,

8) To request compensation for the damage in case they suffer damage due to unlawful processing of their personal data.

3.  Application Method

Pursuant to the first paragraph of Article 13 of KVKK; applications regarding these rights must be submitted to us in writing and signed or by other methods determined by the Personal Data Protection Board (“Board”).

In this context, applications to be made in writing can be submitted to us by printing out this form;

· By the Applicant applying personally by hand,

· Via Notary,

· By registered electronic mail (KEP) address, secure electronic signature, mobile signature or

· By e-mail sent from the electronic mail address previously notified to us by the Applicant and registered in our system.

Below, information specific to written application channels regarding how written applications will be delivered to us is afforded.

Anka Hotel may request additional information to verify the identity of the Applicant in applications made by e-mail by the Applicant and may take necessary measures.

Your applications submitted to us will be answered in writing or electronically within thirty days from the date of notification or receipt of your request, depending on the nature of the request, pursuant to the second paragraph of Article 13 of KVKK.
